J-BusinessDirectory - How do I Edit "add a business" Form

Hi I am having an issue entering multiple companies in J business directory. One Company may have have multiple contact telephone numbers and there is one one field for telephone numbers in the said form. Also, i find myself in a worse situation when i want to add "another location" for a company. Their other locations have different telephone numbers as well but there is no field available to enter that information. I do not want to add this information in "custom attributes" since it will be displayed at the bottom on the contact form and can only viewed in catalog mode. using jbd version 3.6.4 My Question 1) Where can i find the "add a business" form so that i can edit it to show at least 3 fields for telephone numbers. 2) Where can i find the "new location" form so that i can do the same

HI i was able to get the mobile and telephone fields added to the "new location form. How ever i cant see any new location entries on companies which i have entered. I have emailed you a user name and password to check out what may be the cause of this. unfortunately when I replicate the code for an extra field in the add company form, it shows the fields but the extra information does not show up on the front end.
